As part of your rights, Synergie Skin PTY LTD (“Synergie Skin”) believe human DNA samples should not be obtained without the willing consent of the donor.
Unless permitted or required by laws, Synergie Skin, The SkinDNA Company Pty Ltd (the “Laboratory”), and the person/s from which you have obtained this kit (the “Vendor”) will not disclose your personal data to any third parties without your prior consent.
I understand that and consent that:
1. I am being asked to provide a DNA sample to undergo a DNA Skin Analysis and any questions I have relating to this sample collection should be directed to the person/s from which I have obtained this kit (the “Vendor”).
2. Synergie Skin and the Vendor will not collect or receive my DNA sample.
3. My data in the questionnaire will be collected only by the Laboratory
4. My DNA sample will be collected from me using standard techniques, which carry very little risk.
5. Identification of mutations within these genes may assist skin professionals in the selection of appropriate regimens and better skin care management.
6. In some cases, DNA testing is unable to identify an abnormality, even though an abnormality may exist. This may be due to the inability of the technology used to identify certain types of changes (mutations) in genes.
7. Synergie Skin Pty Ltd, SkinDNA Company Pty Ltd, and the person/s from which I have obtained this kit are the only entities having access to my report. To ensure privacy, my report will only have a barcode number for identification. These entities will use all measures possible to protect my information and will not share my information to any other party.
8. My DNA sample may be retained for up to fourteen days after it is processed (for re-testing if necessary), at which time my DNA sample is sent to the isolation bay where it will undergo a safe disposal process.
9. My DNA Data will be de-identified and may be used for statistical purposes and approved research programmes.
10. Participation in genetic testing is completely voluntary and my report are kept strictly confidential until removal.
11. My report will be generated for informational purposes only
I hereby consent to the Laboratory in collection and processing of my DNA, and I fully understand the purpose for which it is being collected.

Our science can demonstrate based on DNA sample data as well as real-time customer survey's that a person between the ages of 55 and 65 with specific known genetic variations is likely to experience measurable outcomes in terms of skin condition.
Using this information, we can look at a 22 year old subject, whose real-time survey can clearly indicate that they have no visible aging issues. However the same genetic variances are present with the younger customer as that found in the 55-65 group.
The more and more samples and survey's, the larger the sample set and stronger the correlation that genetics play big implications with visible interpretations of the skin.
